Initial Functional Needs Assessment

During an Initial Functional Needs Assessment, we look at your capacity to participate in activities of daily living. This helps us to establish goals and treatment priorities to get you back to living your best life as soon as possible.


From this assessment, you will be able to identify which functional activities are meaningful to you, and the ones you want to achieve. From here, our Occupational Therapists identify strategies that can enhance function through rehabilitation, compensation strategies, or even by modifying your environment.


We always come to your home to conduct the assessment, because we want to see how you function in your most familiar environment.

Firstly, we have an interview followed by a performance of activities of daily living and a physical assessment. Depending on your medical needs and condition, we may also perform a cognitive assessment. During the assessment, the Therapist may use any of the following methods:


  • Initial Needs Interview
  • Functional Independence Measure (FIM)
  • Observation of Functional Performance in Everyday Activities
  • Range of Motion Assessment
  • Manual Muscle Testing
  • Pain Scale
  • Cognitive Screening Assessment
